# Der CAC-Score und die Macht der Null: der Herz-Test, der die Krankheit misst, nicht das Risiko

Ein Koronarkalk-Score (CAC) von null gehört zu den stärksten Entwarnungen der Kardiologie: In der MESA-Studie trägt er eine "Garantieperiode" von etwa 3 bis 7 Jahren, und nur rund 8 Prozent der Menschen mit Start-Null überschritten binnen zehn Jahren einen CAC von 100. Er misst verkalkte Plaque direkt, statt nur Risiko zu schätzen, und kann so die Statin-Entscheidung bei beschwerdefreien Erwachsenen um die 40 bis 70 mit mittlerem Risiko kippen. Aber null heißt nicht kein Risiko: Weiche Plaque sieht er nicht, und Rauchen, Diabetes oder ein hoher Lp(a) zählen weiter.

Ein Koronarkalk-Scan zählt die Plaque, die du schon hast. Eine Null kauft dir Jahre Ruhe. Sieh ihn als Tie-Breaker bei mittlerem Risiko, nicht als Test für jeden, und denk dran: weiche Plaque sieht er nicht.
